Free Insurance Claim Documentation Template (Tampa Bay)

A free, carrier-specific documentation template that streamlines water, fire, mold, and storm damage insurance claims for Florida homeowners. Covers State Farm, Allstate, USAA, Citizens, Progressive, and all major Tampa Bay carriers.

Why this matters: Most denied claims are denied not because the damage is not covered, but because the documentation does not match what the carrier wanted. This template fixes that. Use it on day one of any property damage event.

Step 1: Day-of documentation (within the first hour)

The first 60 minutes after damage occurs determine 70% of your claim outcome. Photograph and timestamp everything before you do anything else, including water extraction.

First-hour photo checklist

  • Wide shot of the affected area (one per room)
  • Close-up of the source (burst pipe, water heater, roof leak, etc.)
  • Wet baseboards, drywall edges, and flooring transitions
  • Saturated furniture, electronics, and personal property
  • HVAC vents, return registers, and any visible ductwork
  • Ceiling stains in adjacent rooms (water travels)
  • Time-stamped photo of the thermostat reading
  • Time-stamped photo of any visible mold (do not disturb)

Step 2: The carrier-specific format requirements

Each major Tampa Bay insurance carrier wants documentation submitted in a slightly different format. Submitting in the wrong format does not deny the claim, but it adds 7-14 days to the cycle. Match the format and you skip that delay.

Step 3: The personal property inventory

List every damaged item with: room location, item name, brand, model number, serial number, purchase date (approximate is fine), purchase price (approximate is fine), and replacement cost. The replacement-cost field is what the carrier pays out, not the original purchase price.

Tip from 911 Restoration of Tampa Bay: If the damaged item is over $500 and you do not have a receipt or model number, take a photo of any visible serial-number plate. Most appliances, electronics, and furniture have one underneath, on the back, or in the battery compartment. Carriers accept photographed serial numbers as proof of ownership.

Step 4: The mitigation timeline (most-overlooked section)

Florida insurance regulations require homeowners to mitigate further damage within 14 days of the loss. Carriers can deny portions of a claim if mitigation was not started within that window. Document every step:

  • Date and time you contacted a restoration professional
  • Date and time water extraction began
  • Names and certifications of the technicians on site (IICRC card photos work)
  • Equipment list left on site (dehumidifiers, air movers, HEPA scrubbers) with rental rates
  • Daily moisture-meter readings (most restoration companies provide these in writing)
  • Date dry-out was confirmed
